By booking an appointment, you expressly agree to the following terms and conditions:
Resource or effort commitment
All of Carpe’s services are rigorously selected and carefully executed by our employees to obtain maximum results. With any treatment or medication, unforeseen reactions are possible. Therefore, we do not guarantee the result as it depends on the biological and variable interaction between you and the treatment.
Medical record
All of your medical information is kept in your medical record. This can be done either on paper or electronically. Your medical records always allow Carpe staff to access the information necessary to provide you with the best treatment. All medical records are kept secure.
Only Carpe employees involved in your treatment have access to your medical records.
The Carpe staff uses your information primarily to provide you with the best care possible. Your medical records are important to treating you properly. Accordingly, you should always provide any information that may be important to the Carpe staff (such as, but not limited to: medication, allergies, pregnancy, medical history such as surgeries and illnesses, as well as use of alcohol, etc.). Your medical information may also be used to improve the quality of treatments at Carpe. Only relevant information is used for this purpose, always in anonymous form. No one can then link the information to your personal data.
Carpe wal will not share your medical information with family or confidants unless with your permission. Some exceptions do apply here, such as in the case of minors. In exceptional cases, the law allows us to share your personal information without your consent, such as to investigate a serious crime or to protect a child.
You have the right to review your personal medical records and correct any errors. For a copy of all or part of your medical record, you pay the printing cost of the pages. This is a maximum of 25 euros.
With proof of your identity (copy of identity card), you can obtain the written communication of your medical records via a written, dated and signed request to Carpe. Similarly, you can also ask Carpe to correct the data that would be incorrect, incomplete or not pertinent.
Products used
The Belgian control bodies of the drug agency and the food agency prohibit any explanation of drugs (such as Botox, Dysport, cortisone ointments, etc.) and medical devices such as fillers (Radiesse, Restylane, Teosyal, etc.). Therefore, Carpe cannot provide this information except in a personal consultation with a specialized physician.
Hospitalization insurance and mutual insurance
Hospitalization insurance only intervenes in the costs if there is already reimbursement from a health insurance fund. This means that hospitalization insurance pays only if there is a medical necessity to perform the procedure. Depending on the content of the policy you have taken out with your hospitalization insurance, there may be additional conditions.
For purely aesthetic interventions, however, intervention is unlikely. We know from experience that it is not the first time that insurance companies have promised financial intervention over the phone and gone back on this promise afterwards. So for aesthetic procedures, you should be aware that hospitalization insurance will (most likely) not intervene.
The position of the hospitalization insurance and the mutual insurance company is beyond the responsibility of Carpe, which declines any liability for this.
Payments
All treatments at Carpe are immediately affordable and demandable.
All judicial and/or extrajudicial costs for collection of late payments shall be borne by you. The extrajudicial costs are composed as follows: 10% of the principal sum with a minimum of €25.00. These costs will be charged together with the legal interest, without the need for a notice of default, if you have not paid within 8 days of treatment.
Appointments must be cancelled at least 1 business day in advance. Appointments not cancelled on time will be charged at a flat rate of €50.00 per treatment.
Applicable law and competent courts
Belgian law applies to this site. In case of a dispute, only the courts of the district of Antwerp, Antwerp division have jurisdiction.
